Participation Forms

For the 2023-2024 school year, Canfield Schools will be using Final Forms for collecting student athlete medical and physical information.  Please follow this link. 

Canfield High School and Middle School Athletes, grades 7-12, must complete the physical forms for the 2023-24 academic year.  Fall Sports deadline is August 2nd, 2023.

Yearly physicals are mandatory for participation in all OHSAA sports for the 2023-2024 school year.
